Declaration of consent from the person requiring care

I am an adult or child age 14 or older, and I consent to the use of my data, particularly health-related data, provided by the applicant during the online application process by the Research Service Center for the purpose of evaluating, deciding on, and processing the funding request submitted by the person responsible for my care.
This consent is voluntary. I have the right to withdraw my consent at any time. However, any processing of my data which occurred before consent was withdrawn remains legal and unaffected. I can withdraw my consent at any time by notifying the Research Service Center in writing (forschungsservice@wu.ac.at). If I withdraw my consent, all data processing activities that require my consent will be ended immediately after receipt of my withdrawal notification and the personal data will be deleted. Withdrawal of consent to data processing and the resulting deletion of personal data will result in termination of any decision processes that may have been started with regard to this application.

Data protection declaration
I hereby give the WU Research Service Center consent to process all data provided as part of the online application process for the purpose of assessing, deciding on, and processing research project applications. For the assessment of individual projects, the project applications and the attached additional documents may be sent to external experts or funding bodies and their affiliated entities.
This consent is given freely. I can withdraw my consent to any further data processing at any time, but any processing of my data which occurred before consent was withdrawn remains legal and unaffected. I can withdraw my consent at any time in writing by sending a written notice to the Research Service Center (forschungsservice@wu.ac.at). If I withdraw my consent, all data processing activities that are not permissible without my consent will be stopped immediately upon receipt of my notice of withdrawal of consent, and the data on which these activities were based will be deleted. Withdrawal of consent and the resulting deletion of the personal data lead to the termination of any decision-making procedures that may have been initiated for assessing the application.
